Please complete the following form to request data that is not already published in the Common Data Set. It is our goal to provide prompt and accurate responses to these requests. We offer a turn-around time of 2 to 10 business days to accomplish this goal. However, some requests are more complicated or require some refinement and may take longer than 10 days to complete. Please try to submit your request 2 to 3 weeks prior to your due date. Complete all fields below and submit to the Office of Institutional Effectiveness by clicking “Submit”.

Please be advised that your submitted request is accessible as part of our institution's shared records and may be known to the wider University community. However, the specific outcomes and results of your request are confidential. These will only be shared directly with you, any other individuals included in the submission, and relevant supervisors upon request.

The privacy of student records is governed by the Family Educational Rights and Privacy Act (FERPA). The official university FERPA policy is below. Oglethorpe University is prohibited from providing certain information from your student records to a third party, such as information on grades, billing, tuition and fees assessments, financial aid (including scholarships, grants, work-study, or loan amounts) and other student record information. This restriction applies, but is not limited, to your parents, your guardians, your spouse, or a sponsor. You may, at your discretion, grant the University permission to discuss your record to specific individuals by submitting a student information release authorization.
