Incomplete Grade Form (Student)

 
An incomplete grade may be assigned at the end of a semester when something beyond your control prevents you from completing or submitting your coursework. 

To be eligible for an incomplete you must: 

-Currently have a passing grade in the course

-Be able to complete unfinished work without attending additional courses

-NOT be on Academic Probation. If you are on academic probation and submit this form, the "I", even if approved by the instructor, will revert to a "F" in order to evaluate academic standing as stated in the University Bulletin Section 6.20.2.

The grade "I" has no effect on your GPA and no credit is awarded. Degrees cannot be conferred if there are outstanding I's on your record. 

While not required, it is recommended that you speak with the instructor of the course prior to submitting this form.
